Looking to obtain a copy of your medical records?

We offer two ways to access your medical record:

MyHealthOne Portal:

MyHealthOne consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day.

*Note that some medical records may only be available through our hospital Medical Records office.

Paper Copy of Medical Records:

You can obtain a paper copy of your medical records from Presbyterian/St. Luke's Medical Center through our Health Information Management office. Please note that birth and death certificates are processed through the Colorado Department of Public Health and Environment.

Medical Records Costs

There is a fee for copies of medical records.  Please contact us for more information. 

Records for physician office visits will be handled urgently to insure continuity of care.

Birth and Death Certificates

Certified birth and death certificates are processed and purchased through The Colorado Department of Health, Vital Statistics department.
