Diabetes Self-Management Education & Support
The Diabetes Management Center at Presbyterian/ St. Luke’s Medical Center is dedicated to providing the highest quality diabetes self-management education based on the American Diabetes Association Education recognition standards. Your physician will indicate which of the following class options is best for you.
You must call the Diabetes Management Center at (720) 754-6891 to register for your class. Please contact us Monday–Thursday between 8:00 a.m. and 4:00 p.m. or Friday from 8:00 a.m. to noon. Tell the scheduler which class you need to attend.
New Diagnosis of Type 1 Diabetes
This class is a series of individualized sessions with a Certified Diabetes Educator.
New Diagnosis of Type 2 Diabetes
4-Class Series | 1.5–2 Hours per Class
This class is geared to people who are newly diagnosed with Type 2 diabetes or who have never had diabetes education. Taught by a nurse and/or dietitian as needed based on your individual assessment.
Insulin Pump Therapy
Number of visits is based on individual needs. First class includes an initial consultation and demo.
Our diabetes team can help you start insulin for the first time or make adjustments to the insulin you are already taking. Any recommendations on insulin adjustment will be discussed with and approved by your physician prior to changes being made.
Continuous Self-Blood Glucose Monitoring
A certified diabetes educator will connect the device on the first visit. A second visit is scheduled 7 days later to remove the device and download the results.
One 2-Hour Class
Includes instruction on monitoring blood sugars, diet and medications. At least one follow-up visit is done to assure understanding of the education.
Appointments with a nurse or dietitian can be scheduled for individuals who need a review of their self-management skills.
Medical Nutrition Therapy
Appointments with a dietitian can be scheduled for those with diabetes who would like help with meal planning.
Cost for classes will vary depending on your insurance provider, co-pay and deductible.
